Enrollment Paperwork

Forms & Paperwork

Everything below can go one of two ways: download the PDF and fill it out by hand, or fill it out right here online. Online submissions go straight to RevolutionaryRobotics@proton.me — no printer, no drop-off required. Work through this list top to bottom; it's in the order it actually needs to happen.

Step 01 · Read First

Parent Information Sheet

No fields to fill out — this is the plain-language overview of how the program runs, the practice commitment, and the uniform requirements. Read this before anything else.

Parent & Student Information Meeting

Reference only

Covers Read → Code → Compete, the FIRST LEGO League BIOGLOW season, the practice commitment (2–3x/week), and what parents are responsible for purchasing (team shirt, beret, cargo pants).

Step 02 · Enroll

Registration & Consent

This is split into two pieces right now: the registration info, which is ready to go, and the signed waiver, which is on hold until two open questions come back from our venue and our FLL regional partner.

Registration Information

Live now

Student and parent contact information, medical notes, and photo/video consent. Nothing here is a legal waiver — it's just how we get to know your family and reach you.

A Few More Things

This registers your child with the program. The signed Code of Conduct & Waiver is a separate step below, and we'll follow up once that piece is finalized.

Consent & Code of Conduct Waiver

Pending legal review

Attendance commitment, code of conduct, and assumption-of-risk language. We're holding this off the website on purpose — not stuck, just deliberate.

Download Current PDF
Why this one waits. This section carries our actual liability waiver, and its wording depends on two answers we're waiting on: whether our FIRST LEGO League regional partner requires proof of insurance, and whether It Takes a Village has its own facility waiver we need to fold in. Publishing this online before those come back risks families signing language that changes under them. Print and sign the current PDF version by hand for now if you need to enroll today — the online version goes live here the moment the wording is settled.
Step 03 · Safety Paperwork

Emergency Medical Treatment Authorization

Lets our coaches act quickly if there's a medical emergency and you can't be reached right away.

Emergency Medical Treatment Authorization

Required

Medical conditions, medications, insurance information, and authorization for coaches to seek emergency care if you're unreachable.

Typing your full name below and submitting this form serves as your electronic signature.

Step 04 · Safety Paperwork

Authorized Pickup List

Only the people listed here will be allowed to pick up your child. This matters — take a minute with it.

Authorized Pickup List

Required

List up to five people authorized to pick up your child. If someone not listed here arrives, we contact you directly before releasing your child.

Authorized to Pick Up

Need to add someone not on this list, even for a single day? Notify us in writing before pickup — we won't release a child on a verbal request alone. Typing your name below serves as your electronic signature.

Step 05 · Platform Access

Student Login & Account Information

Paper only — on purpose. Read why below.

Student Login & Account Information

Paper only

Keeps each student's Code.org and Tinkercad login on file so a coach can help with a password reset without losing session time.

This one stays off the website, deliberately. This form asks for your child's account passwords — and its own printed text tells you to keep it in a locked file and "avoid sending passwords by text or email." Turning it into a web form that emails passwords to an inbox would contradict that instruction on the page itself. Print it, fill it by hand, and keep it in a locked binder, exactly as it says.
Step 06 · Literacy Commitment

Library Book Care Pledge

Every Revolutionary carries a library card and reads at least one book a week — this is the promise that comes with borrowing.

Library Book Care Pledge

Required

A short, student-facing pledge to treat borrowed books with care, signed by both student and parent.

Typing a name below serves as an electronic signature.

Step 07 · Read Together

Classroom & Build Room Rules

The full 16-rule standard for the room — worth reading with your child before their first session, not just handed to them.

Classroom & Build Room Rules

Reference only

Organized under Respect, Accountability, Mindset, and Team — the everyday standard, not just the poster version.

Step 08 · Print & Post

10 Rules Poster

The condensed, wall-sized version — 18x24in, built for printing and hanging in the room, not for signing.

Our 10 Rules — Build Room Poster

Print & post

Same standard as the full rules document, condensed to ten lines for a wall poster.

Step 09 · Program Structure

Leadership Council

How the eight student leadership seats work, including the rotating Chief Revolutionary seat — good context once your Revolutionary is settled in.

Leadership Council + Season Roster

Reference only

Every Revolutionary is a leader by design. This explains the eight seats; the second page is a blank roster coaches fill in each season.

For Coaches — Not A Parent Form

FIRST Consent Completion Tracker

This is separate from everything above. It doesn't collect a parent's signature — it's how coaches track that FIRST's own official consent form got completed for every team member.

FIRST® Consent & Release — Completion Tracker

Staff / coach use

FIRST requires its own Consent and Release Form, completed electronically through FIRST's registration dashboard — this tracker just confirms who's been invited and who's finished it. It's not a substitute for FIRST's form.
